[fr] Le Long COVID demeure une maladie complexe dont les mécanismes restent encore partiellement compris. Cette note présente un modèle intégratif proposé à partir des connaissances scientifiques actuelles. Elle décrit le Long COVID comme le résultat probable de l'interaction entre une persistance du SARS-CoV-2, des perturbations biologiques durables, des atteintes localisées de certains organes et une dérégulation des systèmes de contrôle reliant le cerveau au reste du corps. Ces mécanismes biologiques entraînent un handicap fonctionnel souvent disproportionné par rapport aux anomalies visibles sur les examens médicaux. Ils s'accompagnent également de conséquences psychologiques, sociales et existentielles qui influencent l'expérience vécue de la maladie sans en constituer l'origine. Cette approche propose une vision dynamique de l'organisme, fondée sur les interactions permanentes entre les différents systèmes biologiques, la personne et son environnement.
[en] Long COVID remains a complex condition whose underlying mechanisms are only partially understood. This document presents, in accessible language, an integrative model based on current scientific knowledge. It describes Long COVID as the likely result of interactions between the persistence of SARS-CoV-2, sustained biological disturbances, localized organ dysfunction, and dysregulation of the control systems linking the brain with the rest of the body. These biological mechanisms give rise to functional disability that is often disproportionate to the abnormalities detected by conventional medical investigations. They are also accompanied by psychological, social, and existential consequences that shape the lived experience of the illness without constituting its primary cause. This approach offers a dynamic view of the human organism, emphasizing the continuous interactions between biological systems, the individual, and the surrounding environment.
[nl] Long COVID blijft een complexe aandoening waarvan de onderliggende mechanismen nog slechts gedeeltelijk worden begrepen. Deze nota presenteert, in toegankelijke taal, een integratief model dat is opgesteld op basis van de huidige wetenschappelijke kennis. Het beschrijft Long COVID als het waarschijnlijke resultaat van de wisselwerking tussen een persisterende aanwezigheid van SARS-CoV-2, langdurige biologische ontregelingen, plaatselijke aantasting van bepaalde organen en een ontregeling van de controlesystemen die de hersenen met de rest van het lichaam verbinden. Deze biologische mechanismen leiden tot een functionele beperking die vaak buiten verhouding staat tot de afwijkingen die met medische onderzoeken zichtbaar zijn. Daarnaast gaan zij gepaard met psychologische, sociale en existentiële gevolgen die de beleving van de ziekte beïnvloeden, zonder de oorzaak ervan te vormen. Deze benadering biedt een dynamische visie op het organisme, gebaseerd op de voortdurende interacties tussen de verschillende biologische systemen, de persoon en zijn of haar omgeving.
