Abstract :

However, the analysis of organizational metadata exposed within the federal HUB reveals structural inconsistencies: confusion between technical objects and clinical documents, linguistic heterogeneity, absence of explicit hierarchy, and mixing of technical identifiers with conceptual designations.
These findings reflect a deficit in semantic governance affecting clinical readability, the reliability of search functions, and the national comparability of data. Exchange standards cannot compensate for the absence of a structured organizational reference framework.
The implementation of a Federal Organizational Reference Framework, based on explicit metadata registry principles and supported by formal governance, appears to be a strategic condition for the coherence and sustainability of the health information system.
