Abstract :
[en] Systemic lidocaine, widely used for postoperative surgical pain prophylaxis, is increasingly relevant for refractory chronic pain management, particularly visceral pain. This review explores its practical applications in peripheral neuropathies, complex regional pain syndromes, and chronic visceral pain resistant to other recommended molecules. Validated dosages and systemic anti-hyperalgesic and anti-inflammatory mechanisms (prolonged sodium channel blockade, voltage-gated channel modulation) are described and supported by randomized trials and meta-analyses demonstrating the benefit of this indication. Clinical monitoring strategies (continuous ECG, prior hepatic function assessment) and contraindications (arrhythmias, heart failure) are specified for safe use in outpatient or inpatient settings. The complementary role of this molecule in multimodal protocols (antiepileptics, antidepressants, and enhanced recovery programs) is also addressed. This synthesis aims to guide clinicians toward this promising extension of its practical use.
