Abstract :
[en] This multicenter study conducted in three Belgian hospitals evaluated intravenous-to-oral antibiotic switch practices in adult inpatients.
Among 394 intravenous antibiotic prescriptions analyzed, just over half were eligible for an oral switch. However, less than half of these eligible cases were actually converted to oral therapy, and more than half of the switches were either not performed or delayed.
These findings highlight significant gaps in both the implementation and timeliness of IV-to-oral switching, despite well-known clinical, economic, and environmental benefits.
To address this issue, a multidisciplinary team developed an e-learning program including interactive content and clinical cases, aimed at improving and standardizing prescribing practices.
A post-intervention audit is planned to evaluate the impact of this educational strategy.
