[fr] C’est en étudiant trois cas patients décédés avec une anémie idiopathique que le Dr Thomas Addison découvre à l’autopsie des altérations des glandes surrénales. Il en fait un papier, qu’il communique à la South London Medical Society le 15mars 1849 : « On anemia : disease of the suprarenal capsules ». Il remarque également que ces patients ne sont pas pâles mais bronzés : il décrit le terme « mélasme suprarénale ». Plus tard, en 1855 il réunit onze cas semblables dans une monographie de 39 pages : « On The Constitutional And Local Effects Of Disease Of The Supra-Renal Capsules » (1).
Le nom de maladie d’Addison est donné à la maladie correspondant à une insuffisance de production d’hormones par les glandes surrénales. En 1896, William Osler montre qu’on peut traiter des patients souffrant de maladie d’Addison en leur donnant des extraits frais de glandes surrénales provenant d’animaux (2).
Dans cet exposé adressé aux médecines généralistes, nous rappelons, dans un premier temps la physiopathologie de l’insuffisance surrénalienne centrale et périphérique. Nous décrivons ensuite la prise en charge de l’insuffisance surrénalienne. Nous finissons notre exposé par le suivi et la prévention chez le patient avec insuffisance surrénalienne chronique (3). Hotel Tangla, Bruxelles 4.12.2021 [en] While studying three patient cases who died with idiopathic anemia Dr. Thomas Addison discovered at autopsy some alterations in the adrenal glands. He made a paper, which he communicated to the South London Medical Society on 15 March 1849: "On anemia: disease of the suprarenal capsules." He also noted that these patients are not pale but tanned: he then described the term "suprarenal melasma". Later, in 1855, he compiled eleven similar cases in a 39-page monograph: "On The Constitutional And Local Effects Of Disease Of The Supra-Renal Capsules" (1). The name of Addison's disease is given to the disease corresponding to a lack of hormone production by the adrenal glands. In 1896, William Osler showed that patients with Addison's disease could be treated by giving them fresh extracts of adrenal glands from animals (2).
In this presentation addressed to general practitioners, we recall, the pathophysiology of central and peripheral adrenal insufficiency. We then describe the management of adrenal insufficiency. We end our presentation with the follow-up and prevention in patients with chronic adrenal insufficiency (3). Références
