Abstract :
[en] Problem studied: Lung cancer is major public health problem because of its high frequency and high mortality. Its usual treatment consists in taking advantage of radiotherapy, combined or not with chemotherapy, with significant medical side-effects and psycho-affective impact, which harms the quality of life. Even though information is already given to patients, the use of the personal medical images as a tool for therapeutic education has not been studied yet. Population: Adult patients with lung cancer or with lung metastasis, benefiting from external radiotherapy treatment in the radiotherapy department of the CHU of Liège. Method: Experimental study of type “randomized controlled trial”, where the patients were followed over four periods of time and distributed by randomization into an experimental group and a control group. The sample size consisted of a total of 42 patients. As the considered intervention, the patients from the experimental group have viewed their personal CT-Scanner combined with the dosimetry associated with their personal treatment plan, thanks to the “Stone of Orthanc” software, together with explanations from the radiotherapist. The aim of the study was to measure the impact of the intervention according to Kirkpatrick's levels: satisfaction, feeling of involvement, perception of knowledge, feeling of to be prepared, therapeutic adherence and psycho-social impact (anxiety, depression and emotional distress). Two secondary objectives were aimed at the evaluation of the computer program and the indirect impact of the intervention on the nursing staff. Key elements of the critical analysis: The data collection had to be prematurely stopped because of the COVID-19 crisis. Nonetheless, this study highlights several positive, although non-significant results on the side of the experimental group, in particular an improvement of knowledge and a reduction of the psychosocial impacts. As a consequence, the use of medical imaging could bring added value through the individualization of the content and its ability to create and internalize cognitive links, thus developing the skills of the patient.
[fr] Problème examiné : Le cancer du poumon constitue un problème de Santé Publique du fait de sa fréquence et sa mortalité élevée. Le traitement habituel de ces patients est constitué de radiothérapie, combiné ou non avec de la chimiothérapie, avec pour conséquence des effets secondaires et un retentissement psycho-affectif important, ce qui porte atteinte à leur qualité de vie. Même si de l’information est déjà donnée aux patients, l’utilisation de leurs images médicales personnelles comme outil d’éducation thérapeutique n’a pas encore été étudiée. Population étudiée : Patients adultes avec cancer pulmonaire ou avec métastase pulmonaire, bénéficiant d’un traitement par radiothérapie externe dans le service de radiothérapie du CHU de Liège. Méthode envisagée : Étude expérimentale de type « essai contrôlé randomisé », où les patients ont été suivis sur quatre périodes de temps et répartis par randomisation entre un groupe expérimental et un groupe contrôle. L’échantillon était composé d’un total de 42 patients. L’intervention dont a bénéficié le groupe expérimental a consisté en la visualisation de ses propres images de CT-Scanner combinées à la dosimétrie prévue pour leur traitement, via le programme informatique « Stone of Orthanc », accompagnée d’explications du radiothérapeute. Le but de l’étude était de mesurer l’impact de l’intervention selon les niveaux de Kirkpatrick : la satisfaction, le sentiment d’implication, la perception des connaissances, le sentiment de se sentir préparé, l’adhésion thérapeutique et le retentissement psycho-social (anxiété, dépression et détresse émotionnelle). Deux objectifs secondaires visaient l’évaluation de l’outil informatique et l’impact indirect de l’intervention sur le personnel soignant. Éléments clés de l’analyse critique La crise du COVID-19 a prématurément interrompu la collecte des données. Cette étude permet néanmoins de mettre en évidence différents résultats positifs, mais non significatifs du côté du groupe expérimental, notamment en ce qui concerne l’amélioration des connaissances et la diminution du retentissement psycho-social. L’utilisation de l’imagerie médicale pourrait apporter une plus-value par l’individualisation du contenu et sa capacité à créer et internaliser des liens cognitifs, développant ainsi les compétences du patient.
