Comparaison entre les dépenses énergétiques mesurées par Deltatrac II ® et celles mesurées par l’E-COVX ® chez des patients agressés : étude prospective observationnelle
[fr] Objectifs. –
L’apport énergétique à fournir aux sujets agressés est une question complexe et la calorimétrie indirecte (CI) est la méthode de référence pour le déterminer. La dépense énergétique (DE) mesurée par CI, avec le Deltatrac II®, a été comparée aux valeurs mesurées par un nouvel appareil de CI couplé à un ventilateur : l’E-COVX®.
Matériel et méthodes. –
Ces mesures ont été réalisées chez des patients agressés et ventilés, en unités de soins intensifs (USI). La consommation d’O2(VO2), la production de CO2 (VCO2), le quotient respiratoire (QR) et la DE ont été enregistrés. Ces mesures ont été réalisées de manièreconsécutive, aléatoire et pendant une même durée de temps. Des tests-t de Student ont été réalisés pour comparer les résultats ainsi que des graphiques de Bland et Altman.
Résultats. –
Quarante-quatre patients (29 hommes, âgés de 60±15 ans, d’IMC de 25,9 ± 5,3 kg/m2) ont été inclus. Avec le Deltatrac II®,laVO2 était de 244±69 mL/min, la VCO2de 189±47 mL/min, le QR de 0,79±0,08 et la DE de 1648±457 kcal/jour. Les VO2, VCO2et la
DE étaient significativement différentes entre les deux appareils malgré des QR équivalents. Selon nos résultats, le module E-COVX® surestime significativement la DE.
Conclusion. –
Le module E-COVX® constitue une manière pratique pour mesurer la DE en USI. Cependant, ce nouveau dispositif surestime la DE (234 kcal/j) comparé au Deltatrac II ®, méthode de référence. Des améliorations significatives de ces dispositifs sembleraient donc nécessaires avant de pouvoir les utiliser en routine en USI. Material and methods. –
ICU patients on mechanical ventilation underwent measurements, simultaneously connected during 20 min to the ventilator.VO2 consumption, VCO2 production, respiratory quotient (RQ) and EE were recorded. These measures were made successively at random and during a same period. Statistics’ Student t -tests were realized to compare the results. Bland and Altman graphs were also made.
Results. –
Forty-four patients (29 males, aged 6 ±15 years, with a BMI 25.9±5.3 kg/m2) were included. For the Deltatrac II® ,VO2was244±69 mL/min, VCO2 189±47 mL/min, RQ 0.79±0.08 and EE 1648 ±457 kcal/day. VO2, VCO2, and EE differed significantly between the two devices despite similar RQ. By our results, the E-COVX® module significantly overestimates the EE.
Conclusion. –
E-COVX® is a convenient device to measure EE in ICU. However, this new module overestimates EE (234 kcal/day) compared withDeltatrac II®, gold standard method to measure EE. Significant improvements of this news devices seem mandatory before recommending theirclinical use in ICU.
