[en] Ovarian cyst treatment and its efficacy imply to make the diagnosis as early as possible during the end of waiting period or period of reproduction of cows potentially concerned. Moreover, we must keep in mind that during the early postpartum period 38 to 80% of cysts spontaneously regress. The choice of a treatment also depends on criteria and sensitivity level of the identified (follicular or luteinized) cyst. Cysts treatment can be curative or preventive. Give an adequate level of nutrition and reduce the frequency of puerperal diseases during the postpartum period can contribute to reduce the risk of ovarian cysts. Curatively, it's possible manually to rupture or to puncture the ovarian cyst by transvaginal way using or not an ultrasonographic control. Such method can be associated with an hormonal therapy. It's also possible to treat the cyst with hormones. Classically, follicular and luteinised cysts are respectively treated by luteotrope hormones (GnRH, hCG) and by PGF2alpha. As the goal of treatment is to assume a new follicular growth, both cysts can be also treated with progestagens given by intravaginal way. During the last years, different hormonal protocols have been used. Ovsynch has been evaluated and compared with others protocols. Until now, such hormonal associations have a rather limited effect on the pregnancy rates. Moreover, they appear rather expensive. [fr] Le traitement du kyste ovarien et son efficacité supposent un diagnostic aussi précoce que possible chez les vaches en fin de période d'attente ou en période de reproduction. Par ailleurs, il convient de prendre en compte le fait qu'au cours des premières semaines du post-partum 38 à 80 % des kystes sont susceptibles de régresser spontanément. Le choix d'une stratégie thérapeutique doit également dépendre du degré d'exactitude (valeur prédictive) du diagnostic du kyste (folliculaire ou lutéinisé) identifié. L'approche thérapeutique du kyste ovarien peut être préventive ou curative. L'optimisation de la ration alimentaire au cours du post-partum et les mesures visant à réduire la fréquence des affections puerpérales constituent des stratégies préventives dont on ne peut ignorer l'importance. Sur le plan curatif, la ponction, échoguidée ou non, du kyste ovarien associée le cas échéant à un traitement hormonal a été proposée. Le traitement du kyste ovarien peut également être de nature hormonale. Ainsi, classiquement, les kystes folliculaires sont-ils traités au moyen d'hormones lutéotropes (GnRH, hCG) et les kystes folliculaires lutéinisés au moyen des prostaglandines F2 alpha. Le but étant surtout d'assurer une nouvelle croissance folliculaire, le kyste peut également être traité par des progestagènes. Ces dernières années ont vu le recours de plus en plus fréquent à des protocoles associant ces différentes hormones. Le protocole Ovsynch est le plus fréquemment utilisé. Il a fait l'objet de divers essais cliniques et a été comparé à d'autres associations hormonales. À ce jour cependant, force est de reconnaître leur effet limité sur l'amélioration du taux de gestation d'autant que leur coût n'est pas négligeable.
