Abstract :
[en] About 9,500 new breast cancers are diagnosed in Belgium every year. Improvement of our knowledge of altered molecular events leading to the proliferation of tumor cells has resulted in the development of targeted therapies in subgroups of cancers. One of the first validation of targeted therapy is the anti-HER-2 monoclonal antibody trastuzumab (Herceptin) in patients with overexpression of human epidermal growth factor receptor type 2 (HER2) occurring in 20 to 25% of invasive breast carcinoma. Trastuzumab binds the extracellular juxtamembrane domain and is only active in tumor with HER2 gene amplification detected by fluorescence in situ hybridization (FISH). The results from randomized trials have rapidly lead to the approvement of the drug in the metastatic and then in the adjuvant setting. Another targeted therapy, also approved in the treatment of breast cancer, is the monoclonal antibody bevacizumab with an anti-VEGF (Vascular Endothelial Growth Factor) activity. We will review the benefit of these targeted therapies in breast cancer and their role in the treatment of breast cancer.
[fr] Le cancer du sein touche environ 9.500 femmes par an en Belgique. L'amélioration des connaissances concernant les mécanismes moléculaires intervenant dans la prolifération cellulaire a permis le développement de thérapeutiques ciblées. Des progrès importants ont ainsi été observés dans le traitement de certains sous-groupes de cancer. Une des premières validations de cette approche est le développement d'un anticorps anti-HER-2, le trastuzumab (Herceptin®). Il s'agit du premier anticorps monoclonal ciblant la partie extra-membranaire du récepteur HER-2 surexprimé dans 20 à 25 % des cancers du sein au diagnostic. Cette molécule est réservée aux tumeurs présentant une amplification du gène HER-2 prouvée par technique FISH ou aux tumeurs présentant une surexpression 3+ du récepteur déterminée en immunohistochimie. Le bénéfice du traitement a été démontré dans le traitement adjuvant et dans les situations métastatiques du cancer du sein HER-2 positif. Une autre thérapie ciblée validée dans le traitement du cancer du sein métastatique est le bévacizumab (Avastin®), un anticorps monoclonal anti-VEGF (Vascular Endothelial Growth Factor), inhibiteur de l'angiogenèse. Nous allons revoir la place actuelle de ces nouvelles drogues par rapport à la chimiothéra-pie conventionnelle.
