Cette présentation retrace l’histoire de la découverte de la phlorizine, ancêtre des gliflozines. Sa découverte au 19ème siècle par De Koninck et Stas et la démonstration de l’induction d’une glycosurie abaissant la glycémie par von Mering précèdent la découverte du diabète pancréatique par Von Mering et Minkowski. Elle est ensuite utilisée pour conceptualiser la notion de glucotoxicité par l’équipe de DeFronzo.
Dans les années quarante du 20ème siècle, le syndrome de Fanconi et Bickel est décrit chez un garçon de 3 ans, habitant les Alpes. Il a une avec petite taille, une glycosurie, une aminoacidurie, et une glycogenose avec hépatomégalie. En 1997, le syndrome de Fanconi-Bicker est revisité par la génétique, en identifiant des mutations du SGLT2 comme étant responsables du phénotype. La description de ces rares familles avec glycosurie et le développement de modèles murins avec KO du SGLT2 contribueront à améliorer notre compréhension de la physiopathologie du « diabète rénal ».
Enfin, nous décrivons le développement d’inhibiteurs sélectifs (la phlorizine étant non sélective) des cotransporteurs sodium-glucose de type 2 (SGLT2, gliflozines), dans les tubules rénaux, bloquant la réabsorption du glucose. Les gliflozines qui ont dans l’actualité des indications thérapeutiques de plus en plus larges, non seulement dans le diabète de type 2, mais aussi en cardiologie et en néphrologie chez des personnes non diabétiques avec insuffisance cardiaque ou insuffisance rénale, sont un exemple de comment la recherche est alimentée constamment par le présent et le passé.

[en] Summary: This presentation traces the history of the discovery of phlorizine, the ancestor of gliflozins. Its discovery in the 19th century by De Koninck and Stas as well as von Mering's demonstration of the induction of glycosuria and low serum glucose, preceded the discovery of pancreatic diabetes by Von Mering and Minkowski. It was then used to conceptualize the notion of glucotoxicity by DeFronzo's team.
In the forties of the 20th century, Fanconi and Bickel syndrome was described in a 3-year-old boy living in the Alps. It has a small size, glycosuria, aminoaciduria, and glycogenosis with hepatomegaly. In 1997, Fanconi-Bicker syndrome was revisited by genetics, identifying mutations in SGLT2 as being responsible for the phenotype. The description of these rare families with glycosuria and the development of mouse models with SGLT2 knockout will contribute to our understanding of the pathophysiology of "renal diabetes".
Finally, we review the development of selective inhibitors (phlorizine being non-selective) of sodium-glucose type 2 cotransporters (SGLT2, gliflozins), in the renal tubules, blocking glucose reabsorption. Gliflozins, which have increasingly broad therapeutic indications, not only in type 2 diabetes, but also in cardiology and nephrology in non-diabetic people with heart failure or kidney failure, are an example of how research is constantly fueled by the present and the past.
