Abstract :
[en] The effects of a neoadjuvant chemotherapy prior to surgery and / or radiotherapy have been extensively investigated. Up to now, studies failed to give evidence of a survival improvement. We set up a protocol combining cis-platinum, etoposide, fluoro-uracile and ifosfamide.A clear-cut correlation exists between chemosensitivity and radiocurability. In a serie of 181 patients with advanced head and neck cancers, a 75 % chemotherapy response predicts a maximal effect of irradiation in tumours as well as in nodes. A selection tool of patients that might be cured by exclusive radiotherapy without undergoing mutilating surgery has been developed. [fr] Les effets d’une chimiothérapie néo-adjuvante avant chirurgie et / ou radiothérapie ont été largement étudiés. La preuve d’une amélioration de la survie n’a jusqu’à présent pas pu être apportée. Nous avons développé un protocole original combinant cis-platine, étoposide, fluoro-uracile et ifosfamide.Une corrélation étroite est observée entre chimiosensibilité et radiocurabilité. Dans une série de 181 cancers cervico-faciaux avancés, une réponse de 75 % annonce un effet maximal de la radiothérapie tant au niveau tumoral que ganglionnaire.Un critère de sélection de patients guérissables par irradiation exclusive plutôt que par une chirurgie mutilante est ainsi mis en évidence
