Abstract :
[fr] Ce travail s’est intéressé à chercher comment rendre la pratique de la prévention médicalisée des médecins généralistes plus proche d’un objectif d’amélioration de la santé, considérée dans sa définition biopsychosociale, plutôt que de l’amélioration ponctuelle d’indicateurs biomédicaux, tout en maintenant ou améliorant l’équité dans ce domaine. Questions de recherche : • Quelles sont les représentations des médecins généralistes de leurs rôle et actions dans le dispositif préventif existant dans notre pays ? (Chapitre 2). • Quels sont les facteurs qui conditionnent leur implication plus ou moins marquée dans ces domaines ? (Chapitre 3). • Quels sont les modèles de prévention et de promotion de la santé, publiés dans la littérature scientifique en lien avec les pratiques de soins de santé primaire, que l’on peut mobiliser pour poser des hypothèses de travail pour une amélioration de la pratique en prévention ? (Chapitre 4). • Comment articuler la prévention et la promotion de la santé avec la pratique curative ? (Chapitre 5). Quatre études, en lien avec ces questions, sont présentées dans la forme où elles sont publiées (ou en cours de publication), qui contribuent à illustrer cette problématique. Au fil des définitions de la santé, l’importance attribuée aux déterminants structurels, ainsi qu’au contexte économique et social dans lesquels les individus vivent, est allée croissant ; le poids relatif des systèmes de santé dans la détermination du niveau de santé s’est vu progressivement réduit à un rôle plus périphérique. Mais une place reste à prendre dans le domaine de l’éducation des patients, nécessaire pour maîtriser mieux les déterminants individuels de la santé. Les soins primaires et la médecine générale sont adéquatement positionnés pour une approche personnalisée et à grande échelle des populations. Cependant, la formation des médecins ne les prépare pas à mener des actions de promotion de la santé dans le cadre de leur activité curative. La rencontre de généralistes de terrain au cours d’entretiens individuels semi-dirigés a permis un dialogue explorant de multiples facettes de la pratique en prévention. Des facteurs de prédisposition intimement liés à la personnalité des médecins sont apparus déterminants. De façon plus attendue, les limites et difficultés organisationnelles, l’absence quasi-totale de vision collective des médecins envers la population qu’ils desservent, étaient marquantes. Pour avancer dans la compréhension de ce qui conditionne le comportement des généralistes en matière de prévention, au-delà des déclarations, une échelle d’attitude a été construite. L’analyse a identifié quatre facteurs conditionnant l’attitude envers la prévention : évaluation de la performance, adhésion aux recommandations, centrage-patient & collaboration, et partage du pouvoir. Les attitudes des généralistes envers la prévention étaient plus favorables dans les pratiques pluridisciplinaires et moins favorables chez les médecins plus âgés. Une revue narrative de littérature a permis de retrouver douze modèles opérationnels de prévention et promotion de la santé concernant les soins de santé primaires et pouvant servir d’hypothèses de travail pour améliorer la pratique préventive. Ces modèles intègrent progressivement les éléments apportés par les chartes successives de l’OMS sur la promotion de la santé, ainsi que par les rapports de la commission des déterminants sociaux de la santé. Ils indiquent des points de convergence possibles entre la prévention et la promotion de la santé : empowerment individuel des patients, prise en compte de leurs milieux de vie ; participation accrue à des réseaux locaux de professionnels de la santé ; intégration, au sein des mêmes structures, de l’approche individuelle et collective. Les patients en sont pourtant encore fort absents, sauf exception. Les médecins généralistes ont-ils la possibilité matérielle d’entrer plus avant dans une pratique de promotion de la santé pour leurs patients selon les éléments proposés par ces modèles ? L’évolution des systèmes de santé vers un système d’assurance de qualité peut contribuer à la mise en place de certaines des structures et des modes de fonctionnement nécessaires. Le cycle de qualité peut en être un des instruments. Les critères de qualité des soins de santé primaires fournissent le cadre permettant de tracer la voie des réformes à réaliser pour intégrer la prévention et la promotion de la santé. • La globalité des soins impose de considérer l’environnement bio-psycho-social, en intégrant les patients comme des partenaires du processus de soins. • La continuité des soins pose le défi du partage de l’information clinique entre soignants de façon à assurer la qualité des soins, la sécurité des patients et l’équité. • L’intégration des différentes disciplines de la première ligne et des métiers de la promotion de la santé suppose une adaptation des référentiels métiers de chaque discipline. Ces trois défis impliquent des aménagements de la formation des généralistes, et une redéfinition de leurs tâches et compétences propres. Ce travail implique de réunir les cinq composantes du pentagone du partenariat de Boelen autour d’un projet élaboré en commun. La responsabilité sociale des facultés de médecine devrait être valorisée.
[en] The aim of this work was to identify ways to apply preventive health services provided by general practitioners to health improvement, rather than case-by-case improvements of biomedical markers. Health was considered within its bio-psycho-social definition, and equity in health had to be maintained or improved in this process. Research questions: • What are general practitioners (GPs)’ representations of their role and actions in the Belgian preventive framework? (Chapter 2); • What factors can predict GPs’ involvement to a greater or lesser degree in this topic? (Chapter 3); • What are the published operating models about preventive health care and health promotion in primary care that can be used to formulate working hypotheses for the improvement of preventive clinical practice? (Chapter 4). • How to improve the connection between preventive health services, health promotion and the curative approach? (Chapter 5). Four studies are presented that illustrate the topic, as they were published (or under publication). Among the definitions of health, the weight of the structural determinants and the socio-economic context on health levels was stressed, while the weight of the health systems was gradually reduced to a lesser role. But patient education is still expected to help control the individual determinants of health. Primary care and general practice are adequately fit to a tailored approach of large populations. However, the physicians’ vocational training does not address integration of health promotion and curative practice. Meeting field GPs through individual semi-structured interviews allowed for a dialog that explored various aspects of the preventive health services delivery. Predisposing factors closely linked to the physicians’ personality appeared to be determinants. Organisational limitations and difficulties, and almost non-existent collective visions of the practice population were highlighted. To understand what shapes the behaviour of GPs in preventive healthcare delivery beyond statements, an attitude scale was devised. The analysis allowed the statements to be sorted into a four-factor model consisting of performance appraisal, guideline adherence, patient-centeredness & collaboration, and power sharing. The GPs’ attitudes towards preventative service delivery increased with multidisciplinary practice and decreased with seniority in the profession. A narrative literature review retrieved twelve models that address preventive health care and health promotion in primary care; these models helped formulate working hypotheses for the improvement the current situation. The models progressively integrated elements from the health promotion charters of the World Health Organisation and from the Commission on Social Determinants of Health. The following elements valued by health promotion could be integrated to preventive healthcare: empowerment of citizens, addressing community environment; increased involvement in local health professionals’ networks; and the integration of individual and collective approaches within the same health care facilities to simultaneously address individual customisation, efficiency and equity objectives. There was little room for patients in elaborating the models. Do GPs have the opportunity to develop the partnerships that are required to move to health promotion as proposed by these models? The trend toward quality assurance systems among health systems can contribute to the setting of structures and operating modes that are necessary. The quality cycle could be one of the tools used for that purpose. The quality criteria of primary care lead the way to a framework for integrating preventive health care and health promotion. • Comprehensive care addresses the bio-psycho-social environment of patients, considering patients as partners during the health care process. • Continuity of care challenges the managing of clinical data between members of a care team, thereby enhancing quality of care, patient safety and equity. • Integrated care involving caregivers and health promotion professionals will require adaptation of job description reference lists. The challenge created by these three quality criteria entails changes in GPs vocational training and necessitates a new definition of their specific tasks and skills. Such a challenge requires collaborative teamwork of the five members of Boelen’s “partnership pentagon”. The social accountability of medical schools should be fostered.
[nl] In dit proefschrift onderzoeken we hoe de preventiepraktijk van huisartsen meer in de richting van gezondheidsbevordering kan evolueren. Om de gelijkheid in de gezondheidszorg te behouden of te verbeteren, wordt gezondheidsbevordering bekeken vanuit een bio-psychosociaal perspectief. Onderzoeksvragen: • Welk beeld hebben huisartsen van hun rol en activiteiten op vlak van preventie in België? (Hoofdstuk 2). • Welke factoren voorspellen de mate van betrokkenheid van huisartsen in dit kader? (Hoofdstuk 3). • Welke preventie- en gezondheidspromotiemodellen in de eerstelijnsgezondheidszorg zijn beschreven in wetenschappelijke literatuur, en kunnen gehanteerd worden om de preventiepraktijk te verbeteren? (Hoofdstuk 4). • Hoe kan preventie en gezondheidspromotie versterkt worden in relatie tot de curatieve aanpak? (Hoofdstuk 5). In dit proefschrift bespreken we vier studies die bovenstaande onderzoeksvragen illustreren (reeds gepubliceerd of ingediend voor publicatie). In definities van gezondheid wordt steeds meer het belang van structurele determinanten en de economische en sociale context waarin mensen leven, benadrukt. Het relatieve belang van gezondheidssystemen wordt minder belangrijk. Niettemin blijft gezondheidsopvoeding noodzakelijk om individuele determinanten van gezondheid te beïnvloeden. Eerstelijnsgezondheidszorg en huisartsengeneeskunde zijn uitstekend geplaatst voor een gepersonaliseerde aanpak op grote schaal. Nochtans komen initiatieven inzake gezondheidsbevordering in de artsenopleiding te weinig aan bod. Semi-gestructureerde interviews met huisartsen leverden een groot aantal facetten omtrent de preventiepraktijk op. Factoren gelinkt aan de persoonlijkheid van artsen bleken bepalend. Niet onverwachts werden organisatorische knelpunten, en de haast volledige afwezigheid van een collectieve visie van huisartsen over hun patiënten, in de analyse beklemtoond. Om nog beter te begrijpen wat het gedrag van huisartsen inzake preventie bepaalt, werd een attitudeschaal samengesteld. De analyse ervan leverde vier voorwaardenscheppende factoren op: beoordeling van prestatie, zich houden aan richtlijnen, patiëntgerichtheid & samenwerking, en het delen van invloed. Huisartsen in multidisciplinaire praktijken zijn meer preventiegericht. Oudere huisartsen zijn dat veel minder. Een narratief literatuuronderzoek leverde twaalf operationele preventie- en gezondheidspromotiemodellen omtrent de eerstelijnsgezondheidszorg op. Deze modellen fungeerden eveneens als werkhypotheses om de preventiepraktijk te verbeteren. Elementen van diverse charters van de Wereldgezondheidsorganisatie (WGO) over gezondheidspromotie werden progressief geïntegreerd, alsook rapporten van de WGO-Commissie Sociale Determinanten van Gezondheid. De volgende elementen, gevaloriseerd in gezondheidspromotie, kunnen ook geïntegreerd worden in preventieve gezondheidszorg: empowerment van patiënten, belang van omgevingsfactoren, een meer intense aanwezigheid in lokale netwerken van gezondheidsprofessionals, en de integratie van individuele en collectieve benaderingen in gezondheidsvoorzieningen. Patiënten zijn hierin echter nog vaak afwezig. Kunnen huisartsen de omschakeling naar gezondheidspromotie maken, volgens de elementen die voorgesteld worden in deze modellen? De trend naar meer kwaliteitsvol werken kan bijdragen om bepaalde noodzakelijke structuren en methoden van aanpak te installeren. De kwaliteitscyclus kan één van deze instrumenten zijn. De kwaliteitscriteria in de eerstelijnsgezondheidszorg bieden het kader voor een geïntegreerde preventieve gezondheidszorg en gezondheidsbevordering. • Geïntegreerde zorg omvat de bio-psycho-sociale omgeving, waarbij de patiënten tijdens het zorgproces als partners worden betrokken. • De continuïteit van zorg veronderstelt het delen van klinische informatie onder de zorgverleners om zo de kwaliteit van zorg, de veiligheid en gelijkheid van de patiënten te waarborgen. • De integratie van verschillende eerstelijns zorgverleners met professionals in gezondheidspromotie vereist een aanpassing van de diverse jobbeschrijvingen. Deze drie uitdadigingen impliceren de verbetering van de huisartsenopleiding, en een herdefiniëring van hun specifieke taken en competenties. Ook is een samenwerking van de vijf netwerken, zoals omschreven in Boelens ‘Pentagon Partnership’, aangewezen. De sociale verantwoordelijkheid van medische faculteiten moet daarbij gevaloriseerd worden.
