[en] The interest of chronotherapy in the field of arterial hypertension is progressively rising, especially in treated hypertensive patients characterized by a small decrease of their blood pressure during the night, and therefore often presenting a high cardiovascular risk. There are more and more data showing that administration of one antihypertensive drug in the evening (and even aspirin) can improve the blood pressure control during the night and the day/night blood pressure pattern, and so can diminish the level of risk.
The role of chronotherapy also emphasizes the interest of 24 h ambulatory blood pressure monitoring in the management of high risk hypertensive patients. [fr] L’intérêt de la chronothérapie dans le domaine de l’hypertension artérielle gagne progressivement du terrain, notamment chez les patients hypertendus traités dont la baisse de la pression artérielle nocturne est faible, c’est-à-dire souvent les patients à haut risque cardiovasculaire. Il existe, en effet, des arguments de plus en plus nombreux supportant le fait que l’administration le soir d’au moins une molécule antihypertensive, et éventuellement d’une faible dose d’aspirine, puisse améliorer le contrôle de la pression de nuit, restaurer le profil jour/nuit et réduire le niveau de risque.
L’impact de cette chronothérapie met aussi en avant l’intérêt de la mesure ambulatoire de la pression artérielle dans la mise au point des patients hypertendus à plus haut risque.
