[fr] Les lésions tendineuses chroniques sont fréquentes et sont le plus souvent résistantes aux traitements conservateurs disponibles à l'heure actuelle.
Cette revue a comme objectif de regrouper les connaissances actuelles concernant le traitement des lésions tendineuses par plasma riche en plaquettes (PRP). Cette technique bien connue et utilisée depuis une dizaine d'années dans le domaine de la chirurgie, est d'actualité dans le monde du sport, notamment dans le cadre du traitement de tendinopathies réfractaires aux autres thérapeutiques ainsi que d'une récupération fonctionnelle rapide. Cependant, étant donné le caractère récent des traitements par PRP dans le domaine médico-sportif, les études réalisées à ce jour n'ont pas encore permis de reproduire les effets bénéfiques obtenus in vitro. La plupart des études concernant le sujet démontrent une amélioration de la symptomatologie suite au traitement par PRP, mais les nombreux biais introduits en raison par exemple du manque d'effectif, de l'absence de groupe contrôle ou encore du type variable de produit utilisé ne leur confèrent pas un niveau d'évidence assez élevé. A ce jour donc, les résultats obtenus in-vivo ne permettent pas de recommander formellement l'utilisation du PRP dans le cadre du traitement des tendinopathies. L'avenir du PRP devra donc passer par une standardisation dans la fabrication de celui-ci ainsi que par la réalisation de protocoles d'études standards permettant de comparer les futurs travaux. The aim of this review was to gather the actual knowledge concerning the platelet-rich plasma (PRP) treatment in tendon lesion. The interest of this well-known technique, applied since almost 15 years in facial and dental surgery, is growing in the sports world because of its potential ability to treat refractory tendinopathies and to get back quickly a functional use. However, given the relative recent knowledge about PRP in sports medical, the studies could not yet reproduce the beneficial effects
obtained in-vitro. Most studies about the subject indeed show an improvement in symptoms after PRP treatment but, because of the many biases introduced, such as the lack of population, the lack of control group or the many different kind of PRP used, the studies cannot be conferred a high level of evidence. Therefore, there is no evidence of formally advise the use of PRP in the treatment of tendinopathy. So the main research field in the close future will have to determine a standard for the manufacture of PRP and the protocols of the future studies in order to be able to compare them.
