Parkinson’s disease is a neurodegenerative synucleinopathy characterized by the degeneration of neuromelanin-containing dopaminergic neurons and deposition of iron in the substantia nigra (SN). How regional neuromelanin (NM) loss and iron accumulation within specific areas of SN relates to nigro-striatal dysfunction needs to be clarified. We measured dopaminergic function in pre- and post-commissural putamen by [18F]DOPA PET in twenty-three Parkinson’s disease (PD) patients and 23 healthy control (HC) participants in whom NM content and iron load was assessed in medial and lateral SN, respectively by neuromelanin-sensitive and quantitative R2* MRI. Data analysis consisted of voxelwise regressions testing the group effect and its interaction with NM or iron signals. In PD patients, R2* was selectively increased in left lateral SN as compared to healthy participants, suggesting a local accumulation of iron in Parkinson’s disease. By contrast, NM signal differed between PD and HC, without specific regional specificity within SN. Dopaminergic function in posterior putamen decreased as R2* increased in lateral SN, indicating that dopaminergic function impairment progresses with iron accumulation in the SN. Dopaminergic function was also positively correlated with NM signal in lateral SN, indicating that dopaminergic function impairment progresses with depigmentation in the SN. A complex relationship was detected between R2* in the lateral SN and NM signal in the medial substantia nigra. In conclusion, multimodal imaging reveals regionally-specific relationships between iron accumulation and depigmentation within the SN of Parkinson’s disease and provides in vivo insights in its neuropathology. Rapid-onset dystonia-parkinsonism also known as DYT12-ATP1A3 is an extremely rare neurological disease. Patients develop dystonia, bradykinesia, postural instability, dysarthria, and dysphagia. Injection of botulinum toxin is the first-choice treatment for focal dystonia. We report the case of a 14-year-old patient diagnosed with rapid-onset dystonia-parkinsonism who was scheduled for injection of botulinum toxin in his upper limbs under general anesthesia. To our knowledge, there is no previous report about the anesthetic management of patients with rapid-onset dystonia-parkinsonism. [less ▲]

The few published reports of ATP1A3-mutated patients who received pallidal surgery (deep brain stimulation or pallidotomy targeting the globus pallidus pars interna [GPi-DBS]) are disappointing as all implanted patients failed to improve. We report the case of a 7-years-old boy who developed severe recurrent episodes of ataxia. Following the persistence of intrathecal pleocytosis and oligoclonal bands, auto-immune workup was performed and anti-neurochondrin antibodies in the cerebro-spinal fluid were discovered. Other investigations returned normal, no oncologic accompaniments were found. Clinical examination identified cerebellar ataxia, cervical dystonia, choreic movements of the upper limbs, action tremor, opsoclonus, dysarthria and akathisis (Video 1). Cognitive affective cerebellar syndrome, including neuropsychological impairment associated with significant emotional lability, was also found. Initial corticosteroid therapy (intravenous methylprednisolone 30mg/kg for 3 days) resulted in significant improvement. However, following the third relapse, a long-term corticotherapy with monthly intravenous methylprednisolone 500mg/m² was initiated. After 8 months, in order to avoid side effects, this treatment was replaced by mycophenolic acid. No relapse was observed since then. The child currently keeps slightly ataxic gait as well as significant cognitive impairment. Autoimmune etiologies of movement disorders are increasingly recognized, even in children. Unexplained repeated episodes of ataxia of subacute onset may require a workup with comprehensive neural IgG screening, especially since immune therapies seem more effective than what is observed in adults.
