We report a rare type of stress fracture in a young athlete, great hope of belgian table tennis. Although this type of fracture is not rare in the broad sense of the term, a stress fracture located at the distal femoral physeal is exceptional. Such a complication must be quickly detected because of growth disorders and impact on sports performances that this may cause. On the basis of an anamnesis and detailed clinical observations, we describe the physiopathology as well as the diagnostic and therapeutic management of this pathology. [less ▲]

Osteochondritis dissecans is a necrosis of the subchondral bone and the overlying cartilage. In the advanced stages, a fragment can detache and can be found free in intra-articular, responsible for blockages. This pathology affects adolescents, especially men; a notion of intense physical activity is often found. The most interested joint is the knee, especially the medial femoral condyle. Symptoms (pain, blockages) and clinicals signs (edema, crepitations) worsen during physical activity. The treatment begins with an exhaustive clinical examination, follows the iconographic assessment (standard radiographs, MRI), conservatives treatments generally predominate surgicals techniques. But, these last are sometimes inevitables. Rehabilitation is essential in order to avoid residual functional impotence. In this context, we propose the use of an isokinetic test, which makes it possible to personalize the management and adaptation of the rehabilitation to the needs of each one. The long term follow-up is essential to monitor and prevent osteoarthritis. [less ▲]

The spine has been the subject of extensive clinical research since it is the source of many painful complaints. However, there is little scientific evidence concerning the therapeutic proposals. During the course of life, the intervertebral disc degenerates, which over time diminishes its damping capacity and facilitates the expulsion of the nucleus pulposus through the annulus fibrosus. The degeneration of the intervertebral disc (DDI) is the origin of some back pain and various specific treatments have been developed. These include the infiltration at the center of the intervertebral disc of plasma rich platelet (PRP), composed of multiple growth factors which act on the disc degeneration. This treatment is recent and less invasive than surgery. Preliminary results seem promising. However, many gray areas and several parameters remained to be clarified. In an attempt to do this, a literature review was conducted based on bibliographic databases Pubmed, Medline and Scopus® using the following Mesh terms : PRP, platelet-rich plasma, intradiscal disc degeneration, disc, intradiscal, discogenic. This analysis reveals that at the present time, no reported study has a sufficient perspective to judge the effectiveness of the infiltration of PRP. Early harvest results will be used to set the limits of this treatment. Accordingly, it is therefore currently recommended to introduce PRP injection as a complementary solution to comprehensive care of the spine. Future research will need to generate randomized controlled studies including comparing the results with conservative treatment and measure the cost-benefit relationship. [less ▲]

The present study assessed the ability of suckling rats to express short-term behavioral sensitization to cocaine prior to weaning. Rat pups, aged either 3, 5, 10, 12, 17 or 19 days at the beginning of the experiment, were placed in a chamber after daily injection with cocaine (7.5 or 15 mg/kg. i.p.) for either 2 or 4 consecutive days, and were tested for behavioral responsiveness to cocaine in the same chamber 24 h later (at either 7, 14 or 21 days of age). Such a short post-treatment interval was adopted, along with a consistent pairing of the testing context with the drug effect and a sensitive technique of behavioral measurement (video recording), in order to maximize the possibility of detecting any cocaine sensitization. Locomotion was sensitized at all ages, after both regimens in 14-day-old pups, but solely after 2 injections in 21- and 4 injections in 7-day-old pups. Sensitization was also expressed via behaviors specific to each age. Four cocaine injections augmented cocaine-induced uncoordinated movements of head, paws and body (horizontal activity) in 7-day-old pups, and mouth movements in 14-day-old pups. In 21-day-old pups, sensitization was dose- and regimen-dependently expressed via adult-like stereotyped head movements. In neonatal 7-day-old pups, cocaine sensitization was also visible as reductions in immobility (both injection regimens). Contrary to previous studies, these results indicate that, given the use of an appropriate methodology, short-term sensitization to the motoric effects of cocaine can be expressed by suckling rats prior to weaning, even after relatively short regimens of daily injections.
