BACKGROUND: To describe the medico-economic characteristics of Crohn's disease (CD), we implemented a multicenter study in France. METHODS: From 2004 to 2006, disease severity states, direct (hospital and extra hospital) and indirect costs were prospectively collected over 1 year in patients with CD naive from anti-tumor necrosis factor alpha (infliximab) at inclusion. Economic valorization was performed from the French Social Insurance perspective, and a statistical modeling over 10 years was performed. RESULTS: In 341 patients, the mean total costs of management were &OV0556;6024 per year (&OV0556;4675 for direct costs). As compared to patients in remission, costs were 4 to 6 times higher in patients in an active period and 19 times higher for patients requiring surgery (SURG). The most important expense items were medical and surgical hospitalizations (56% of total costs), including cost of infliximab (36% of hospitalization costs, i.e., 20% of total costs), indirect costs (22%), and drugs (11%). The statistical modeling over 10 years showed that most of the clinical course was spent in drug-responsive state (54%) with 26% of costs or in remission (32%) with 11% of costs; time spent in a SURG state was small (3.2%) but generated 48% of total costs. CONCLUSIONS: Before the introduction of self-injectable anti-tumor necrosis factor alpha, the most important expenses were supported by hospitalizations, explaining why the most costly states were for patients requiring SURG or dependent on inhospital administrated drugs. Projected data show that most time is spent in a stabilized state with appropriate treatments or in remission, and that costs associated with SURG are high. [less ▲]

L’administration fiscale peut imposer une cotisation distincte sur commissions secrètes notamment lorsque des bénéfices dissimulés ne se retrouvent pas parmi les éléments du patrimoine de la société qui fait l’objet d’un contrôle. À défaut d’une définition légale, les contours de ces deux conditions d’application, à savoir d’une part, l’existence de bénéfices dissimulés et, d’autre part, le fait que ces bénéfices ne se retrouvent pas parmi les éléments du patrimoine de la société concernée, demeurent particulièrement flous. Dans la pratique, une kyrielle de positions existe sur la question de savoir quand une cotisation distincte sur commissions secrètes peut être effectivement appliquée. Il est un point en particulier qui divise littéralement tant les auteurs que les cours et tribunaux : des dépenses non admises peuvent-elles être considérées comme des bénéfices dissimulés ? Le 20 février 2014, la Cour de cassation a rendu une décision nuancée à ce propos, en distinguant deux catégories de dépenses non admises selon que la raison qui justifie le rejet des dépenses en cause est ou non l’absence de contreprestations réelles. Cette jurisprudence et les conséquences qu’elle a pu avoir sur l’application de la cotisation distincte sur commissions secrètes font l’objet d’un examen détaillé dans la présente contribution. La loi-programme du 19 décembre 2014 a sensiblement modifié l’article 219 du C.I.R. 1992. Le nouvel article 219, alinéa 5 prévoit que les bénéfices dissimulés ne sont soumis à la cotisation distincte sur commissions secrètes que dans le cas où ils ne sont pas le résultat d'un rejet de frais professionnels. Ce nouveau texte est-il en phase avec la jurisprudence de la Cour de cassation ? Enfin et surtout, le nouveau régime de la cotisation distincte sur commissions secrètes peut-il mettre fin aux nombreuses controverses et être le vecteur de la sécurité juridique tant promise qu’espérée ? Cotoneaster horizontalis DECAISNE, a Rosaceae coming from Asia, was introduced in Belgium as an ornamental species. Different sides of the invasion were studied, from the distribution of the species and the invasion status, the impacts, to management methods. To describe the invasion status, information was gathered about the occurrence and the distribution of the species in Belgium by compiling various databases. The naturalization status in calcareous grasslands, high-value habitats recognized as biodiversity hotspots, was assessed. Occurrence, population status, preferred habitats, invaded communities, growth rate and fruiting capacity were characterized. Once naturalization established, we investigated its impacts on calcareous grasslands communities, individual species and pollinators. C. horizontalis occurred in seven of the nine studied calcareous grasslands, with densities from 0.34 to 10 individuals/ha. In the most invaded sites, an ongoing colonization process was suggested by a high proportion of small individuals. Moreover, 3-year-old individuals already showed fruiting capacities. Germination rate reached about 30%. Significant impacts on habitat structure and vegetation were showed. Community structure and composition changes were demonstrated in invaded sites by decreasing species richness and diversity. As calcareous grasslands are priority Natura2000 habitats, measures must be taken to control the spread of C. horizontalis. For this purpose, we performed management tests to identify the most efficient techniques that would help the restoration of calcareous grasslands. Both mechanical and chemical management techniques were tested. Methods choice was driven by the environmental constraints of calcareous grasslands, which require highly selective techniques, and by species characteristics. Tested methods were cuttings, stump burning, spraying, cutting plus glyphosate application on stump and glyphosate application on stem basis. Efficiency, cost, feasibility and side-effects on ecosystems were estimated. Management tests are still ongoing but some results have already been drawn: spraying and single cutting are not efficient, the last one generating production of numerous sprouts. Finding the best management method will not be sufficient to stop the invasion. C. horizontalis is still present in 53% of the sale catalogs of horticulturists in Southern Belgium. As long as the species is used in the vicinity of calcareous grasslands, propagules are continuously provided, jeoparding management actions. [less ▲]

Les cultivars de cotonniers génétiquement modifi és sont aux portes de lʼAfrique francophone après avoir été massivement adoptés en Afrique du Sud. Afi n dʼanticiper les effets des cotonniers Bt sur le petit paysannat, nous nous livrons à une analyse simple de la rentabilité de cette culture sur base de résultats obtenus en Afrique du Sud et de données recueillies par des collègues au Mali. Si la commercialisation du cotonnier Bt peut se justifi er clairement sur le plan technique par la crainte dʼune amplifi cation de la résistance des chenilles carpophages aux insecticides, la rentabilité de cette innovation est par contre bien moins certaine. Le profi t réalisé par le paysan dépendra essentiellement du niveau du rendement conditionné par des aléas dʼordre climatique, agronomiques et environnementaux et du prix de la licence technologique à charge du paysan. Pour un prix dʼachat du coton-graine de 210 FCFA, le prix de 25 USD par ha semble être la limite au-delà de laquelle le paysan court un risque fi nancier. Dans le contexte actuel de baisse des prix dʼachat, la contribution fi nancière du paysan à toute redevance technologique est questionnable. Au niveau de lʼensemble de la fi lière, le succès de lʼadoption du cotonnier Bt en Afrique francophone sera probablement fonction de plusieurs éléments : 1) la prévention des résistances à la toxine Bt ; 2) le renforcement du contrôle intégré des insectes piqueurs ; 3) la modernisation du système de production de semences et 4) lʼamélioration du système de vulgarisation et de suivi agricole. Le cotonnier Bt doit être considéré comme un élément du management intégré de la culture et pas comme la solution unique à lʼallègement de la pauvreté. [less ▲]

Recent studies have suggested that coinfusion of mesenchymal stem cells (MSCs) the day of hematopoietic cell transplantation (HCT) might promote engraftment and prevent graft-versus-host disease (GVHD) after myeloablative allogeneic HCT. This prompted us to investigate in a pilot study whether MSC infusion before HCT could allow nonmyeloablative (NMA) HCT (a transplant strategy based nearly exclusively on graft-versus-tumor effects for tumor eradication) from HLA-mismatched donors to be performed safely. Twenty patients with hematologic malignancies were given MSCs from third party unrelated donors 30-120 minutes before peripheral blood stem cells (PBSCs) from HLA-mismatched unrelated donors, after conditioning with 2 Gy total body irradiation (TBI) and fludarabine. The primary endpoint was safety, defined as a 100-day incidence of nonrelapse mortality (NRM) <35%. One patient had primary graft rejection, whereas the remaining 19 patients had sustained engraftment. The 100-day cumulative incidence of grade II-IV acute GVHD (aGVHD) was 35%, whereas 65% of the patients experienced moderate/severe chronic GVHD (cGVHD). One-year NRM (10%), relapse (30%), overall survival (OS) (80%) and progression-free survival (PFS) (60%), and 1-year incidence of death from GVHD or infection with GVHD (10%) were encouraging. These figures compare favorably with those observed in a historic group of 16 patients given HLA-mismatched PBSCs (but no MSCs) after NMA conditioning, which had a 1-year incidence of NRM of 37% (P = .02), a 1-year incidence of relapse of 25% (NS), a 1-year OS and PFS of 44% (P = .02), and 38% (P = .1), respectively, and a 1-year rate of death from GVHD or infection with GVHD of 31% (P = .04). In conclusion, our data suggest that HLA-mismatched NMA HCT with MSC coinfusion appeared to be safe. [less ▲]
