Retroperitoneal fibrosis is an uncommon inflammatory disease which is characterized by the development of a fibrous process that surrounds the major vessels and organs located within the retroperitoneum. Isolated as an idiopathic expression or associated with several other diseases in its secondary form, retroperitoneal fibrosis remains relatively of rare occurrence and as a result, is devoid of either diagnostic or therapeutic definitive recommendations. We report the observation of one case admitted in the emergency room for abdominal pain induced by left hydronephrosis. Radiographic exploration with contrast-enhanced studies evidenced retroperitoneal fibrosis causing left ureteric obstruction. Therapeutic approach was based on corticosteroids and ureteral drainage which prevented recurrence while fibrosis was almost reversed six months later. [less ▲]

Over recent years, anti-TNFalpha have been used to treat rheumatoid arthritis. The principal secondary effect of anti-TNFalpha is tuberculosis infections. Another paradoxical effect, previously less well understood, is the development of sarcoid-like granulomatous reactions. We report the case of a 36 year old woman who had been treated for 9 years with anti-TNF alpha. She developed a pulmonary sarcoid-like gra-nulomatosis, complication that is rare but not exceptional in patients treated with TNF-blockers. Discontinuation of anti TNF usually led to recovery. It has been suggested that these reactions mainly occur with etanercept, but this requires further confirmation. [less ▲]

When the tumor is found on the skeleton, it generally involves a long bone. X-ray does not permit the differenciation of epithelioid hemangioma from other,malignant, vascular tumors. We describe a patient suffering from infection of the upper respiratory tract, who was treated with a dose of 4 x 4 g of piperacillin over 10 days. Two days after the end of the treatment, she developed jaundice and had elevated alkaline phosphatase, gammaglutamyltransferase and transaminases. After exclusion of viral hepatitis, a vascular problem, and gall stone disease, the possibility of piperacillin-induced hepatitis was discussed. Lymphocyte transformation test for piperacillin was positive, suggesting an immunological mechanism for the observed hepatopathy. The patient was discharged a few days after in good clinical condition and with reduced liver values. Cholestasis gradually decreased but was detectable for several weeks. The patient had a full clinical and biochemical recovery after 2 months. We conclude that short-term therapy with piperacillin can lead to the same type of hepatopathy as described for amoxycillin/clavulanic acid or antistaphylococcal penicillins. Positive lymphocyte transformation is compatible with an immunological mechanism. [less ▲]

We report the case of a 70-year old man who presented the classical cutaneous signs of chronic arsenicism : palmo-plantar keratoses and lesions of Bowen's disease. Within a few months, the patient developed multiple large superficial basal cell carcinomas on the trunk and several invasive squamous cell carcinomas. A pulmonary cancer with cerebral metastases was also identified. The complications of chronic arsenicism are nowadays rarely encountered. The cutaneous changes should be recongized because they allow to reveal the etiology of some internal lesions. Indeed, the notion of past arsenicism is often missing in the patient records. The recognition of the cutaneous warning signs allows to focus on the overall cancer risk of these patients. [less ▲]
