Les troubles du sommeil sont fréquents chez l’enfant et leurs origines sont diverses. Ces troubles ont leur ancrage dans le développement neurophysiologique et psychoaffectif de l’enfant. Les enfants souffrant de troubles du spectre autistique présentent un déficit de sommeil, qui peut durer jusqu’à l’adolescence. Les troubles du spectre autistique comportent des comorbidités multiples, dont des altérations du sommeil. Ils sont importants à caractériser dans cette population, car ils peuvent avoir un impact négatif sur les comportements diurnes. Par ailleurs, ils entrainent souvent des difficultés au sein de la famille. La prise en charge doit avant tout prendre en compte le contexte environnemental afin d’améliorer les habitudes et l’hygiène du sommeil. Des questionnaires ainsi que des agendas de sommeil ont été remplis par des parents afin de démontrer l’importance d’une évaluation et d’une prise en compte des troubles du sommeil chez les enfants souffrant d’autisme en consultation pédopsychiatrique. In line with Evidence-based medicine and more recently with the emergence of Evidence-based practice in psychology, what are the current trends in medical prescriptions in child psychiatry? The use of psychotropic drugs for children and adolescents is constantly increasing, although most pharmacological prescriptions in child psychiatry result in off-label drug use. This article aims at understanding the legal and scientific framework that regulates the prescription of psychopharmacological treatments for children and adolescents. In the absence of clear therapeutic indications, which scientific data should be used to lead our prescribing practice? This topic is illustrated on a common case in child psychiatry: the prescription of medicines to treat psychotic disorders. [less ▲]

THIS PAPER HAS FOR STARTING POINT THE PRESENTATION OF A CLINICAL CASE OF A SELF-HARMING ADOLESCENT. FROM THAT PERSPECTIVE, WE WILL INVESTIGATE THE LITERATURE TO FIND WHAT WE KNOW ABOUT THE LINKS BETWEEN SELF-HARMING AND BORDERLINE PERSONALITY DISORDER (BDP), IN ORDER TO PROVIDE AN OVERALL PICTURE OF THIS TYPE OF PATIENT. IN THIS WAY, THE CASE PRESENTATION SERVES AS AN ILLUSTRATION FOR THE THEORETICAL ANALYSIS. AMONG THE BDP PATIENTS, 70 TO 80% OF THEM EXHIBIT SELF-INJURIOUS BEHAVIORS, BUT WITHOUT ANY PAIN IN 50 TO 60% OF THE CASES. THIS LACK OF PAIN IS A SPECIFICITY OF BDP PATIENTS AND WOULD BE RELATED TO THE AFFECT REGULATION PROCESS. SELF-HARMING WOULD THEREBY HAVE THE EFFECT TO REDUCING STRESS. THIS IS A DISTURBING PHENOMENON BECAUSE THE LACK OF PAIN INCREASES THE RISK OF A HIGHER INCIDENCE OF SELF-HARMING BEHAVIORS AND A GREATER NUMBER OF SUICIDE ATTEMPTS.
