The XXL survey currently covers two 25 sq. deg. patches with XMM observations of ~10ks. We summarise the scientific results associated with the first release of the XXL data set, that occurred mid 2016. We review several arguments for increasing the survey depth to 40 ks during the next decade of XMM operations. X-ray (z<2) cluster, (z<4) AGN and cosmic background survey science will then benefit from an extraordinary data reservoir. This, combined with deep multi-$\lambda$ observations, will lead to solid standalone cosmological constraints and provide a wealth of information on the formation and evolution of AGN, clusters and the X-ray background. In particular, it will offer a unique opportunity to pinpoint the z>1 cluster density. It will eventually constitute a reference study and an ideal calibration field for the upcoming eROSITA and Euclid missions. [less ▲]

The European recommendations for the management of infective endocarditis have been updated in 2015. The therapeutic modalities depend on both a better prevention in high-risk population, a faster diagnosis through new approaches in medical imaging, and interdisciplinary collaboration by an " endocarditis team. " Treatment involves antimicrobial therapy which can be complex, initially empirical and then targeted, depending on local infectious epidemiology, and whether the infringement affects a native or a prosthetic valve. The surgical indications are identified in the individual patient, based on the response to medical therapy, on complications (heart failure, uncontrolled infection and prevention of embolic events) the germ, the type of valve involved, and of course an integration with the patient's comorbidities. Context. The quest for the cosmological parameters that describe our universe continues to motivate the scientific community to undertake very large survey initiatives across the electromagnetic spectrum. Over the past two decades, the Chandra and XMM-Newton observatories have supported numerous studies of X-ray-selected clusters of galaxies, active galactic nuclei (AGNs), and the X-ray background. The present paper is the first in a series reporting results of the XXL-XMM survey; it comes at a time when the Planck mission results are being finalised. Aims. We present the XXL Survey, the largest XMM programme totaling some 6.9 Ms to date and involving an international consortium of roughly 100 members. The XXL Survey covers two extragalactic areas of 25 deg2 each at a point-source sensitivity of ∼5 × 10-15 erg s-1 cm-2 in the [0.5-2] keV band (completeness limit). The survey's main goals are to provide constraints on the dark energy equation of state from the space-time distribution of clusters of galaxies and to serve as a pathfinder for future, wide-area X-ray missions. We review science objectives, including cluster studies, AGN evolution, and large-scale structure, that are being conducted with the support of approximately 30 follow-up programmes. Methods. We describe the 542 XMM observations along with the associated multi-λ and numerical simulation programmes. We give a detailed account of the X-ray processing steps and describe innovative tools being developed for the cosmological analysis. Results. The paper provides a thorough evaluation of the X-ray data, including quality controls, photon statistics, exposure and background maps, and sky coverage. Source catalogue construction and multi-λ associations are briefly described. This material will be the basis for the calculation of the cluster and AGN selection functions, critical elements of the cosmological and science analyses. Conclusions. The XXL multi-λ data set will have a unique lasting legacy value for cosmological and extragalactic studies and will serve as a calibration resource for future dark energy studies with clusters and other X-ray selected sources. With the present article, we release the XMM XXL photon and smoothed images along with the corresponding exposure maps. © ESO, 2016. [less ▲]

Pharmaceutical research is looking for new alternatives to manage the metabolic disorders associated with obesity. In this context, 11beta hydroxysteroid dehydrogenase type 1 (11HSD1) represents an interesting target. Indeed, this enzyme activates the transformation of inactive cortisone into active cortisol in various tissues. Therefore, it may be responsible for a local hypercortisolism, in adipose tissue and/or liver, which may be implicated in the pathogenesis of abdominal obesity, metabolic syndrome and type 2 diabetes. Thus, the inhibition of 11HSD1 may represent a potential pharmacological target and an innovative therapeutic goal. Several studies in both animals and humans led to the development of specific 11HSD1 inhibitors, with promising preliminary results. Indeed, a reduction in insulin resistance and significant improvements in carbohydrate and lipid profiles have been reported. The present article describes the rationale that led to the development of specific 11HSD1 inhibitors and briefly reports the first results obtained with these molecules, which may become a new class of antidiabetic agents in the future. La 11β-hydroxystéroïde déshydrogénase de type 1 (11HSD1), qui convertit la cortisone (inactive) en cortisol (actif) dans les tissus cibles, est surexprimée dans certains tissus en présence d’une obésité, surtout abdominale, et pourrait jouer un rôle dans les anomalies métaboliques associées. Des inhibiteurs non sélectifs de la 11HSD1 ont déjà été testés chez l’homme et se sont révélés peu efficaces. Le développement de nouveaux inhibiteurs, plus puissants et plus sélectifs, est en cours. Des résultats préliminaires obtenus avec divers inhibiteurs sont prometteurs, avec une diminution de l’insulinorésistance, une amélioration de la tolérance au glucose ou du contrôle glycémique et une correction de certaines anomalies liées au syndrome métabolique. Si ces résultats se confirment, les inhibiteurs sélectifs de la 11HSD1 pourraient représenter à l’avenir une nouvelle classe pharmacologique d’antidiabétiques oraux. Cet article donne au lecteur un aperçu des différentes étapes de réflexion et d’expérimentations humaines qui ont mené au développement d’inhibiteurs de la 11HSD1 à visée thérapeutique. [less ▲]

Abdominal obesity plays a key role in the development of metabolic syndrome. Similarities between metabolic syndrome and Cushing disease suggest that excessive local tissue exposition to glucocorticoids, despite normal circulating plasma levels, might contribute to the pathophysiology of metabolic syndrome. To this respect, 11β-hydroxysteroid dehydrogenase type 1 (11HSD1), the enzyme which converts cortisone (inactive) to cortisol (active) in target tissues, raises much interest. Indeed, several studies showed both increased expression and activity of this enzyme in adipose tissues in presence of obesity. Even more striking, experimental data in rodents showed a direct link between increased 11HSD1 activity and the development of metabolic abnormalities. Furthermore, studies in mice KO for 11HSD1 confirmed the potential of inhibiting this enzyme to attenuate metabolic abnormalities related to visceral adiposity. Selective inhibitors of 11HSD1 are currently in development, and preliminary results obtained in rodents appear promising, with significant improvements in glucose and lipid profiles. The development of potent and selective 11HSD1 inhibitors may open new prospects in the treatment of metabolic syndrome or type 2 diabetes associated with abdominal obesity in humans. [less ▲]

Burbidge et al. (\cite {burbidge}) argue that the observed number of quasar pairs with small angular separations and different redshifts (typically Delta theta <= 5 arcsec and Delta z > 0.1 ) is not compatible with a random distribution of quasars over the sky. After a brief review of all known quasar pairs with different redshifts, we show by means of very simple calculations that the probability of finding the three accepted pairs accidentally is of the order of 10%. We conclude that, under realistic hypotheses, the observed number of quasar pairs with different redshifts is not unlikely. We also present arguments showing that gravitational lensing biases are probably not strong enough to significantly increase the expected number of quasar pairs. The failure to detect with HST a secondary lensed image of the background quasar near the foreground one in these three pairs supports this view. [less ▲]
