Aims of the study The aim of the study was to investigate the influence of fatigue on stride biomechanics measured by a portable accelerometer, the MyotestRun. Opportunities of the protocol allowed additional analysis on the device reliability and sensitivity. Methods Thirty-two male subjects, divided in two groups (runners and control), were involved in the study and achieved a treadmill progressive VO2max test; repeated run at constant velocity on a track and a field test at 90% of the VvO2max until exhaustion. The biomechanical measurements obtained from the MyotestRun allow exploring the reliability, the influence of speed, the differences between treadmill and track stride and the influence of fatigue. Results Results show an excellent reproducibility of the MyotestRun values, except for the asymmetry of the stride. No significant stride difference appears between treadmill and track conditions. The progressive test shows that all the parameters evolve with speed increment, confirming the sensitivity of the MyotestRun. The study shows changes in the stride with the fatigue, such as an increase of the contact time, a decrease of the force. However any change in the stride frequency is observed. If the study shows that the fatigue pattern is more constant in the runners group it also highlights adaptations to fatigue that can be very different from one subject to another. Conclusion This study confirmed that MyotestRun is a useful device in the stride analysis. The influence of fatigue on the stride is in accordance with most of the studies while confirming very important interindividual differences. [less ▲]

Infliximab is an IgG1κ monoclonal antibody against tumor necrosis factor-α (anti-TNFα) who have significantly improved the management of patients with Inflammatory Bowel Diseases (IBD), but with an increased risk of opportunistic infection, especially tuberculosis (TB). We report a case of a 54 years-old man with an ileal Crohn’s disease (CD) diagnosed in July 2014 (Montreal classification at diagnosed: A2L1B1p-). Patient was an active smoker and was treated by budesonide for several flares. Two years after diagnosis, since he has been hospitalised for subocclusion treated with methylprednisolone, combination therapy by mercaptopurine and anti-TNFα was discussed for steroid dependence. Patient was screened for tuberculosis (TB). A latent tuberculosis (LTB) was highlighted based on a positive interferon-gamma release assay (QuantiFERON TB) without clinical, microbiological and radiological evidence of active disease. A nine-month course of isoniazid (INH) 300 mg/day was started 4 weeks before the combination therapy with thiopurine/infliximab at the recommended doses. This treatment allowed a clinical remission and thiopurine could be stopped. Six months after completing INH for LTB and 1 month after cessation of thiopurine, patient was admitted in our emergency department for fever, dyspnoea and cough. Chest tomography showed large lymphadenopathies in the right pulmonary hilum and in the mediastinum. A fibroscopy with a bronchoalveolar lavage (with looking for bacillus of Koch) and an endobronchial ultrasound with biopsies were performed, both negative. Finally, a diagnosis of ganglionic tuberculosis was settled by a positive microscopic exam (auramine coloration) on an excised cervical lymphadenopathy. No resistance to usual anti-tuberculosis treatment was highlighted. Infliximab was immediately discontinued and anti-tuberculosis 4-drug regimen was initiated. Patient received Vedolizumab as IBD treatment 1 month later and he did not present any recurrence of TB infection or new flare of Crohn's disease up until now. This case report emphasizes that chemoprophylaxis for LTB does not completely protect against reactivation of TB in IBD patients treated by anti-TNF. It could be useful to study risk factors for reactivation of LTB in this treated by biologics population (monotherapy or combination therapy) to establish a case-by-case monitoring. [less ▲]

Running biomechanics and its evolution that occurs over intensive trials are widely studied, but few studies have focused on the reproducibility of stride evolution in these runs. The purpose of this investigation was to assess the reproducibility of changes in eight biomechanical variables during exhaustive runs, using three-dimensional analysis. Ten male athletes (age: 23 ± 4 years; maximal oxygen uptake: 57.5 ± 4.4 ml02·min-1·kg-1; maximal aerobic speed: 19.3 ± 0.8 km·h-1) performed a maximal treadmill test. Between 3 to 10 days later, they started a series of three time-to-exhaustion trials at 90% of the individual maximal aerobic speed, seven days apart. During these trials eight biomechanical variables were recorded over a 20-s period every 4 min until exhaustion. The evolution of a variable over a trial was represented as the slope of the linear regression of these variables over time. Reproducibility was assessed with intraclass correlation coefficients and variability was quantified as standard error of measurement. Changes in five variables (swing duration, stride frequency, step length, centre of gravity vertical and lateral amplitude) showed moderate to good reproducibility (0.48 ≤ ICC ≤ 0.72), while changes in stance duration, reactivity and foot orientation showed poor reproducibility (-0.71 ≤ ICC ≤ 0.04). Fatigue-induced changes in stride biomechanics do not follow a reproducible course across the board; however, several variables do show satisfactory stability: swing duration, stride frequency, step length and centre of gravity shift. BACKGROUND: Various clinical trials show benefits of physical training offered during and / or after breast cancer treatments. However, given the variety of side effects that may be encountered, physical training could be combined with psychological, relational and social guidance. This kind of multidisciplinary program, just like their long-term effects, have been little studied so far. Therefore, the objective of our study is to determine the benefits at 3, 6, 12 and 24 months of a three-month exercise and education program among women after breast cancer treatment. MATERIALS AND METHODS: Two hundred and nine outpatients who have been treated for a primary breast carcinoma were divided into a control group (n=106) and an experimental group (n=103) which has benefited from a three-month rehabilitation program including physical training and psycho-educational sessions. The assessments, performed before the program and at 3, 6, 12 and 24 months after inclusion, included validated questionnaires on quality of life and symptoms. RESULTS: The analyses show an improvement in quality of life and symptoms following the exercise and education program within the experimental group and a maintenance of these improvements during the two years of follow-up. These improvements are significantly superior to those presented in the control group, demonstrating the programs benefits. CONCLUSIONS: This trial identifies the benefits of a well detailed three-month exercise and education program over a follow-up of 24 months among women after breast cancer treatment. [less ▲]

BACKGROUND: Different clinical trials show beneficial effects of physical training offered during and / or after breast cancer treatment. However, given the variety of side effects that may be encountered, physical training could be combined with psychological, relational and social guidance. This kind of multidisciplinary program has been little studied so far. AIM: To determine the benefits of a three-month multidisciplinary rehabilitation program among women after breast cancer treatment. DESIGN: Controlled no-randomized trial. SETTING: University for outcomes, University Hospital Center for interventions. POPULATION: Two hundred and nine outpatients who have been treated for a primary breast carcinoma. METHODS: Patients were divided into a control group (n=106) and an experimental group (n=103) which has benefited from a rehabilitation program of three months including physical training and psycho-educational sessions. The assessments, performed before and after the program, included functional assessments ("Sit and Reach Test", maximal incremental exercise test and "Six-Minute Walk Test"), body composition measurements (body mass index (BMI) and body fat percentage) and a questionnaire (European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30). RESULTS: After three months, flexibility, walking distance and all parameters measured during the maximal incremental exercise, except maximal heart rate, were significantly improved in the experimental group. The body fat percentage was significantly decreased and a significant improvement was observed for perceived health status (quality of life), functional role, emotional state, physical, cognitive and social functions and for most symptoms. In the control group, most of these improvements didn't appear and a significant increase in BMI and body fat percentage was observed. CONCLUSIONS: This trial identifies the benefits of a well detailed multidisciplinary rehabilitation program, including physical re-conditioning and psycho-educational sessions, with important improvements in functional capacity, body composition and the majority of functions and symptoms among women after breast cancer treatment. CLINICAL REHABILITATION IMPACT: Through its results, this study could contribute to the development of hospital quality standards for oncologic rehabilitation. Physiotherapists can efficiently propose this kind of multidisciplinary rehabilitation program. [less ▲]

BACKGROUND: Physical exercises in addition to standard care (SC) in patients with Parkinson’s disease (PD) are now common practice in many care units. However, exercises can cover a wide range of interventions, and the specific effects of different interventions still deserve to be further investigated. AIM: To compare the effects of 12 weeks of two different types of physical exercises with SC in patients suffering from PD. DESIGN: Pseudo-randomized controlled trial. SETTING: University laboratory for outcomes, University Hospital Centre for interventions. POPULATION: Fifty-two outpatients suffering from mild to moderate PD at baseline. METHODS: Participants were allocated to 3 groups: the strength training (ST) group performed individualized upper and lower limbs strength training, the aerobic training (AE) group performed tailored gradual aerobic cycling, and the third group received SC. The effects of the interventions on body function were assessed by measuring isokinetic concentric peak torque for knee extension and flexion, peak oxygen consumption (VO2peak) and peak work load (PWL) during an incremental maximal cycling test. Changes in mobility were evaluated from spatial-temporal gait features measured by mean of an accelerometer system and the six-minute walk distance (6mwd) test. We used questionnaires to estimate health-related quality of life and habitual physical activity. RESULTS: No significant changes in any outcome measures occurred in the SC group. More than 80% of the participants adequately completed the AE and the ST interventions. The ST group significantly improved all peak torque measures (p≤0.01), except knee extension in the least affected side (p=0.13). This group also improved the PWL (p=0.009) and 6mwd (p=0.03). The AE group improved the VO2peak (p=0.02) and PWL (p<0.001). CONCLUSION: Physical fitness in patients with PD rapidly improved in compliance with training specificities, but better fitness hardly translated into better mobility and health-related quality of life. CLINICAL REHABILITATION IMPACT: Physiotherapists can efficiently propose physical conditioning to patients with mild to moderate PD, but these interventions are insufficient to improve gait and participation. Notwithstanding, ST is an efficient intervention for improving walking capacity. [less ▲]

Objectif : Le cancer du sein ainsi que ses traitements sont à l'origine de nombreux effets secondaires indésirables, notamment sur la fonction physique. Cette étude a pour objectif de déterminer les bénéfices éventuels d’un programme de revalidation multidisciplinaire sur la santé physique des femmes ayant été traitées pour un cancer du sein et d'évaluer l'influence de certaines caractéristiques individuelles initiales sur la réponse du groupe expérimental au programme de revalidation. Matériel et méthodes : L'étude constitue un essai clinique contrôlé non-randomisé évaluant une population totale de 209 patientes, recrutées sur base du volontariat et réparties en deux groupes, soit un groupe contrôle (n = 106) et un groupe expérimental (n = 103). Ce dernier a bénéficié d’une revalidation de trois mois comprenant un entraînement physique supervisé à raison de trois séances par semaine et diverses sessions psycho-éducatives. Pour l'ensemble des participantes, des évaluations constituées d'une part de mesures physiques et fonctionnelles et d'autre part de mesures de composition corporelle ont été programmées à 0 et 3 mois. Celles-ci ont inclus un test de souplesse ("Sit and Reach"), un test d'effort maximal sur bicyclette ergométrique avec suivi cardio-respiratoire, un test de marche de six minutes et la mesure de l'indice de masse corporelle et du pourcentage de graisse corporelle. Afin d'affiner l'analyse, trois groupes ont également été constitués au sein du groupe expérimental à partir des caractéristiques individuelles de thérapie adjuvante (présence de chimiothérapie ou non), de chirurgie (mastectomie ou tumorectomie) et de délai de fin de traitement (délai ≤ 4 mois ou ≥ 8 mois). Résultats : Initialement, toutes les patientes présentent un état de déconditionnement physique, non influencé par le délai de fin de traitement ou par le type de chirurgie, mais bien influencé par la présence d'une chimiothérapie puisque la VO2max apparaît significativement plus faible dans ce cas. Au terme des trois mois, la souplesse (p < 0,0001), la consommation maximale d'oxygène (p < 0,0001), la puissance maximale aérobie (p < 0,0001) et la distance de marche en six minutes (p < 0,0001) s’améliorent significativement au sein du groupe expérimental. Au sein du groupe contrôle, aucune amélioration n'est constatée, excepté pour la distance de marche (p = 0,0031). Par ailleurs, une augmentation significative de l'indice de masse corporelle (p = 0,034) et du pourcentage de graisse corporelle (p = 0,034) est observée au sein du groupe contrôle alors que ce paramètre diminue significativement au sein du groupe expérimental (p = 0,037). Enfin, toutes les patientes du groupe expérimental présentent un même profil de progression quelles que soient leurs caractéristiques individuelles initiales. Conclusion : Cette étude démontre ainsi les effets bénéfiques, tant sur le plan physique que de la composition corporelle, d’une prise en charge multidisciplinaire chez des femmes ayant été traitées pour le cancer du sein. BACKGROUND: Regular physical activity is an important health factor, but intense physical stress can increase the risk of heart disease. OBJECTIVE: Our aim was to determine the potential cardiac repercussions of, and the oxidative stress resulting from a maximal eccentric isokinetic exercise and a 1-hour treadmill run at 75% ˙V O2 max (maximal exercise done 6 weeks before). METHODS: Twelve young sedentary healthy subjects randomly performed two tests separated by 6 weeks: 1) 3 sets of 30 maximal eccentric isokinetic contractions of the quadriceps; 2) a 1-hour running on treadmill at 75% ˙V O2 max. We drew blood samples just before each exercise (T1), and just after (T2), 3 hours after (T3), and 24 hours after (T4) the end of each exercise to measure cardiac and oxidative stress biomarkers. RESULTS: In the running group, we observed significant differences for myoglobin (T3: 145 ± 80 μg/L), creatinine kinase (T4: 593 ± 350 mg/L), oxidized glutathione (T2: 22 ± 15.6 μmol/L), and highly sensitive cardiac troponin T, (T3: 0.051 ± 0.038 ng/mL). In the isokinetic group, we observed significant differences for myoglobin (T3:1419 ± 2533 mg/L), creatine kinase (3303 ± 7159 mg/L), and oxidized glutathione (T4:24 ± 14 μmol/L). Between isokinetic exercise and running, we observed significant differences for uric acid (p < 0.05, running > eccentric), myoglobin (p < 0.05, ditto), NT-proBNP (p < 0.05, ditto), hsTnT (p < 0.01, ditto), and oxidized glutathione (p < 0.05). CONCLUSIONS: As cardiac biomarkers appear practically unmodified after the isokinetic exercise, despite the considerable oxidative stress, we suggest that the application of intense maximal eccentric isokinetic exercise, when indicated, should be safe for most patients including those whose cardiac status is unknown. On the other hand, the increase in cardiac biomarkers observed after running, could reflect leakage of these biomarkers from the cytosolic pool of cardiac cells, linked to membrane damage, rather than the result of a major injury and hence running is supposed to be a safe practice. However, since sudden death during running has been previously described, assesment of the cardiac biomarkers and a follow-up by a sport doctor is important especially if there is a cardiac family history. We compared six 3D methods, OptoGait, and Myotest Run for the determination of contact time for runners with different foot strike patterns. Twenty male participants were divided into two groups: the heel group, who attack the ground with the heel (n = 12), and the toe group, who attack the ground with the middle/front of the foot (n = 8). They performed trials at speeds of 8 km/h then 16 km/h. To detect foot strike, the use of peak velocity of 3D markers located on the heel, the fifth metatarsal, and the great toe provided the best results for both groups. To detect the toe off, the minimum vertical position of a 3D marker placed in line with the great toe gave the most satisfactory results for both groups. In this way, the values of contact time measured with the 3D methods are consistent. Values measured with OptoGait appear consistent too, while those of the Myotest Run underestimate the contact time for both speeds. 3D analysis provides interesting opportunities for calculation of contact time for both rearfoot and forefoot runners, using specific peak velocities to determine foot strike and marker displacement to determine toe off. Background : Breast cancer is the most frequent cancer in women, affecting 8 women out of 100. With early detection and improved treatments, the number of deaths linked to this disease has declined. However, the disease and its treatments are at the origin of many undesirable side effects such as fatigue, anxiety, weight gain and sleep disorders. The objective of this study is to determine the psychological benefits of a multidisciplinary rehabilitation program among women treated for breast cancer (on average six months after the end of the adjuvant radiotherapy or chimiotherapy). Material and methods : 122 patients were included into a control group (n = 61) and a treated group (n = 61). All participants were submitted to evaluations before the beginning of the trial and after three months. These evaluations included different questionnaires exploring the quality of life, anxiety, depression and various functions and other symptoms related to cancer (EORTC QLQ-C30, EQ-5D, STAI, HADS) and functional assessments. The control group has received no intervention unlike the treated group that received a three-month rehabilitation including supervised physical training (90 min) with three times a week and various psycho-educational sessions (120 min) once a week. Results : After three months, the health status (quality of life) (p < 0,0001), the functional role (p = 0,031), emotional state (p < 0,0001) and physical (p = 0,0045), cognitive (p = 0,0027) and social functions (p = 0,0018) improve significantly in the treated group. This observation also applies to symptoms of fatigue (p < 0,0001), insomnia (p < 0,0001), pain (p = 0,002), dyspnea (p = 0,009), loss of appetite (p = 0,04), anxiety (p < 0,0001) and depression (p < 0,0001) as well as physical parameters obtained through functional assessments. In the control group, these improvements do not appear. Conclusions : This study shows the feasibility and psychological benefits of such a multidisciplinary oncological rehabilitation program in women after their treatments for breast cancer. Additional studies are needed to know the optimal time of beginning (during treatments or after them) and the optimal management time for this support. [less ▲]

The control group has received no intervention unlike the treated group that received a three-month rehabilitation including supervised physical training (90 min) with three times a week and various psycho-educational sessions (120 min) once a week. Results : After three months, maximal oxygen consumption (p < 0,0001), maximal aerobic power (p < 0,0001), peak of ventilation (p < 0,0001) and time to exhaustion (p = 0,0055) during the maximal incremental exercise protocol improve significantly in the treated group. This observation also applies to flexibility (p < 0,0001), walking distance in six minutes (p < 0,0001) and different physical and psychological parameters obtained through questionnaires. In the control group, these improvements do not appear and a significant increase in body mass index (p = 0,032) and body fat percentage (p = 0,034) is observed while these data remain constant in the treated group. Conclusions : This study shows the feasibility and physical benefits of such a multidisciplinary oncological rehabilitation program in women after their treatments for breast cancer. Additional studies are needed to know the optimal time of beginning (during treatments or after them) and the optimal management time for this support. [less ▲]

Le cancer constitue une maladie fréquente en Belgique puisqu'un homme sur trois et une femme sur quatre sont confrontés à la maladie avant l'âge de septante-cinq ans (Fondation Registre du Cancer, 2008). La détection précoce et l'amélioration des traitements du cancer, généralement une combinaison de chirurgie, radiothérapie, chimiothérapie, hormonothérapie et thérapie ciblée, ont permis une augmentation du taux de survie. Cependant, ces traitements peuvent être à l'origine de nombreux effets secondaires, non seulement précoces, mais également tardifs, réduisant la qualité de vie. De nombreux programmes de réhabilitation après cancer sont basés sur la psychothérapie et le support social uniquement, or ceux-ci n'agissent généralement pas sur les problèmes physiques encourus par les patients tels que la fatigue, la prise de poids et la diminution des capacités fonctionnelles. C'est pourquoi nous émettons l'hypothèse qu'une revalidation physique, associée à des séances psycho-éducatives et constituant ainsi une prise en charge multidisciplinaire, permettrait d'améliorer d'autant plus la qualité de vie et le bien-être tant physique que psychologique des patients ayant été traités pour un cancer. Objectives.– To assess the effects of an aerobic training for people with Parkinson's disease (PD). Methods.– Fourteen patients with Hoehn & Yahr stage [1] < III of PD were allocated to either 3 months of twice-weekly aerobic training (AT group; n = 8), or a control group (C group; n = 6). AT consisted in progressive stationary bike training between 50 and 70% of peak work load (PWL). Maximal incremental test on a cycloergometer was performed before and after rehabilitation. Anxiety, depression and quality of life were assessed using questionnaires. An Anova for repeated measures was used for statistical analysis. Results.– There was a significant “group by time” effect for peak work load (+21% for patients of AT group vs –9% for people of C group). Heart rate 2 minutes after test completion was also significantly decreased in AT group (post hoc analysis, P < 0.01). Patients of AT group also increased their ratio of predicted VO2 peak (+17%) but in a non-significant way. No significant changes in anxiety, depression or quality of life could be highlighted. Conclusions.– This study showed significant improvement of PWL and faster recovery in patients with PD of the AT. Significant effect on VO2 peak could need a more intense training program. [less ▲]
